OTHER INFORMATION
Access for the Disabled
We are continually reviewing access for the disabled and make any reasonable changes we can in order
to satisfy the Disability Discrimination Act.
Our main surgery at Selby is far more accessible and 'user friendly' for the disabled than our branch surgeries. There is parking near the main entrance for the disabled, access in to the surgery is made via automatic doors, there are no steps to hinder access and all consultations take place on the ground floor. For those patients with hearing difficulties, we have a hearing loop covering the reception desk, prescription counter and waiting room.
Access at our branch surgeries is not as good and whilst parking causes no problem, there are a number of small steps at the entrance to the surgeries. All patient services are however on the ground floor. If you encounter problems of any nature please contact our receptionists for advice and guidance.
Patient transport services are provided to help patients get to their appointment if they have have a clinical condition that means they can't get there safely on their own. This is a separate service from emergency ambulances, which deal with emergency patient care.
You are entitled to hospital transport if you:
need a stretcher for a journey or are confined to a wheelchair
need oxygen or other medical gases during the journey
have a mental health problem, learning disability, speech, sight or hearing difficulty which means you can't use public transport
can't walk without relying on another person or walking aid
have side effects from your medical condition which stop you using public transport
If you are eligible, to book transport please telephone the Contact Centre on 0870 600 0668. The centre is open 8.00 am to 6.00 pm Monday to Friday (it is not open on public holidays)
If you want to see all or a part of your record held by the Practice, you should make a written request to the Practice Manager. Arrangements will be made for you to see your record, normally within 21 days of your request. You are entitled to receive a copy of the information but should note that a charge will usually be made, as a contribution to the costs of administration, photocopying and/or printing, up to a maximum of £50.

Breast Screening Programme
Women aged between 50 and 70 are now routinely invited for free breast screening every three years. In Selby this is done via a mobile unit that works from the car park at Selby Hospital.

'Carers'
If you are a 'Carer' and look after someone or if you are cared for by someone it is important that you let us know. We may be able to provide you with information to assist you in this role, to offer you support or to direct you to organisations who may be able to provide financial assistance.
The Selby & York Carers Centre can also provide you with free and valuable advice, they can be contacted on 01904 - 724466 (24 hour answerphone)
Change
of Details
It is vital that you inform us of any change in your personal details. This is
particularly important should you change your name, address or telephone number.
This means that you receive information promptly, as well as ensuring that you
are recalled for screening programmes at the appropriate time.
Should you change address it may be that you move out of our practice area. Should this be the case it is important that you re register with a local practice as soon as possible. If you are unsure about whether you have to change practices or how to go about this, we would be happy to advise you.
If we identify that you are residing outside the practice area we will write to you recommending that you register at a practice nearer your home. This letter will also give details of the who to contact to get details of surgeries on your locality.

Chaperones
It is sometimes necessary for the doctor to perform examinations which patients may find embarrassing. At this practice, it is the policy of the doctors and nurses to offer you a chaperone for this type of examination.
Our chaperones are all trained people and will usually be a nurse or another doctor.
If the doctor or nurse offers you a chaperone, it is your decision as to whether you accept, you do not have to
be chaperoned if you do not wish to be. Alternatively you may arrange another appointment with a doctor of your choice
or at a time when you can bring a friend or relative.
However, there may be occasional circumstances when the doctor or nurse may be unable to proceed without a
chaperone. Because our chaperones are nurses and doctors who are also running their own surgeries, they may not be
immediately available to chaperone you should you request this. If this is the case, you may have a short wait before
your examination can be carried out, or you may be asked to book another appointment at a time when a chaperone can
be arranged.
You should not avoid an examination if the doctor believes that you need it.

Drug Misuse
The misuse of drugs is becoming increasingly common in the UK. A free, non prescribing counselling service for all issues relating to drugs is available from Compass Drug Resource Scheme.
This service is available to parents, carers who may require support and information.

The emergency contraception Morning After Pill is available FREE without a prescription from Selby Hospital Minor Injuries Unit
Interpreters
For those patients who speak little or no English we have access to interpreters. Should an interpreter be required then this will be done via telephone. The consultation will therefore be 3 -way, as such all such consultations can take longer than the usual 10 minutes.
For those patients who use British Sign Language (BSL) as their first language we have access to BSL interpreters through the Royal National Institute for the Deaf. Unfortunately, due to the shortage of such interpreters we have to book their services up to 6 weeks in advance.
All interpreters are obliged to ensure all details discussed during any consultation remain confidential.
Transplants are one of the most miraculous achievements of modern medicine but they depend entirely on the generosity of donors and their families who are willing to make this life saving gift to others.
There is a critical shortage of donated organs for transplant. More than 8,000 people in the UK need a an organ transplant to save or dramatically improve their lives. The generosity of donors means that nearly 3,000 people every year take on a new lease of life. But still around 400 people die each year while waiting for a transplant.
Nine out of ten people support organ donation, but only 22% of then have registered their wishes on the NHS Organ Donor Register.

Removal from the Practice List
In rare circumstances it may be that the practice feels the need to remove patients from the practice list. This may be a result of unacceptable behaviour (as outlined above) crime or deception or indeed distance from the practice. It is important to note that we would NOT normally remove a patient from the list on clinical matters, patient choice or complaints.
Should this course of action be deemed necessary then the following steps will be taken:
Patients will be contacted by letter advising of the problem and asking them to consider a meeting to discuss matters. This would usually be with the GP concerned and the Practice Manager
This meeting will attempt to explain to the patient the nature of the problem and to elicit the patients perspective of the situation
If the discussion fails to resolve the problem then it may be suggested that the patient sees another GP within the practice or alternatively registers with another practice within the area.
If the situation remains unresolved the practice will write to Selby & York PCT advising of the reasons for the removal. The patient will also be informed in line with clauses 181-189 of the new GMS Contract. This letter will explain that the patient will not be left without a GP and that the patients family will not automatically be removed.
Selby Watercolour
Society
Along the corridors at Beech Tree Surgery are numerous paintings that are provided by Selby
Watercolour Society. These paintings can be purchased: the title, artists name and price of each
painting is located alongside each picture.
Sickness Certificates/Sick Notes
A sick note is not needed if you are of work for less than 7 days (including weekends). This period is covered by a self certificate available from your employer. If your absence is for longer than 7 days then you will need to make an appointment with a doctor.
As a general rule, unless there were exceptional circumstances sick notes will not be back dated.
Training
The practice has been an accredited training practice since 1994. As part of this the practice must assist in the training of GP Registrars. GP Registrars are with the practice for a period of 6 months before they move on to another part of their training. Dr Mark Williams is the dedicated trainer within the practice and is responsible for the Registrar whilst they are with the practice. As part of their training each Registrar must video consultations with patients. Patients who have appointments when videoing is to take place will be advised of this at the time of making their appointment and will be asked for their written informed consent. All patient have the right to ask that their consultations are NOT videoed. Click here to find details of our current Registrar.
From September 2006 the practice will also be helping with the training of medical students from the Hull-York Medical School (HYMS). The students will be nearing the final stages of their training & will already have considerable experience in talking to patients in other surgeries & hospitals.
Talking to patients and learning about conditions and experiences is a vital part of the students training and we hope to be able to help them with this at the surgery. We hope that you as patients will feel able to help them also.
Patients are most likely to encounter medical students if they attend on a Tuesday & book an appointment with either Dr Jackson, Dr Scott or Dr Williams. However, other doctors, nurses or midwives may also be accompanied by one of two medical students from time to time.
We will ask patients in advance whether they mind seeing a medical student who will be supervised by one of the regular GP's. We might patients whether they mind having medical students observe their consultation with a GP. Patients have the right to refuse this and their care will be in no way affected as a result
We hope that patients will feel able to help with this important contribution to training future doctors
Zero Tolerance
The Practice closely follows the NHS Zero Tolerance campaign launched by the Health Minister in 1999.
The doctors ands staff are here to help you and want top provide the best
possible standard of care. Occasionally doctors and staff are faced with verbal
abuse and even threats of physical violence. This
type of unacceptable behaviour will not be tolerated.
Unacceptable behaviour is any behaviour, which you would not like to receive yourself, and includes swearing, shouting, banging fists and threatened or actual violence. Incidents of this type are not only of concern to practice staff but also to other patients waiting to be seen.
Should this type of incident occur, the person (s) concerned should be aware that such incidents will be recorded and that they may be removed from the practice list.
The police may also be called to assist in certain circumstances.
