Chronic Disease Management Clinics

 

All recent research has shown that conditions that are longstanding and require regular monitoring are best looked after in a clinic setting. The clinics we run are designed to ensure that  we provide then most appropriate and up to date care and that you are reviewed regularly.

 

Patients suffering from Heart Disease/Stroke, Diabetes, Asthma, Chronic Obstructive Pulmonary Disease (Chronic Bronchitis) and Hypertension will be called automatically for a review. This letter will advise when an appointment is due to be made and what other action may be required before the date of the appointment.  An explanatory leaflet will also be posted out that will give further information. All clinics take place at our main surgery in Selby.  

 

Your time at the clinic is to monitor a specific condition only and we ask that you make a separate appointment for any other medical conditions you may have.

Other Dedicated Clinics

 

Blood Tests/Phlebotomy

Patients who need to give a sample of blood should attend outpatients at Selby War Memorial Hospital. The clinic operates between 8.30 am & 12.30 pm Monday to Friday and on Monday afternoons between 1.00 pm & 4.20 pm. 

 

From Monday 10 July 2006 an appointment ONLY system will operate for this service. 

To make an appointment please telephone 01904 - 724310, between 9.00 am and 12.00 noon

Five minutes is allocated to each appointment and appointments can be made up to 2 weeks in advance.

 

 

Cervical Smear Tests
These important checks facilitate early detection of cancer of the cervix (neck of the womb). All female patients between the ages of 25 and 65 should be tested every 3 years, or more regularly if advised for certain conditions. Younger patients may also be recommended for testing. Appointments can be made with the Nurse; results can take up to 8 weeks to get back from the laboratory.

Child Development Checks
Checks are made on all children during their pre-school years (see below)

 

8 weeks: Child seen by GP (parents need to make this appointment themselves)
2 years: Child seen by Health Visitor for developmental assessment

 

Letters are sent by the Practice inviting parents to make  an appointment for their child

for a 8 week check.

 

 

Family Planning and Emergency Contraception
We provide a full contraceptive service in normal surgery hours. Emergency contraception is available from the Nurses as well as Doctors, and it is important to be seen within 72 hours of any potential contraceptive failure or omission. This service is also available from the Minor Injuries Unit at Selby War Memorial Hospital.

 

 

Flu Clinics

The Department of Health recommend that certain groups of the population have a flu vaccination each year:

All patients eligible for a flu vaccination will be contacted by letter during the month of October. Due to the large number of patients to be vaccinated, dedicated clinics are set up that run from our main surgery in Selby on a Saturday's. These appointments are for FLU VACCINATIONS ONLY. Appointments for flu vaccinations at our branch surgeries of Carlton & Riccall are not usually available until late November/early December each year.

 

Appointments for the 2007 flu campaign can now be made.  

    

Health Promotion
All of our Nurses can give help and advice on lifestyle problems. They can check Blood Pressure and advise about smoking, alcohol intake, diet and exercise.

 

 

Maternity
The majority of doctors within the practice run weekly antenatal clinics. The Midwife runs her own clinic alongside these.

 

 

Minor Surgery
All the Partners undertake minor surgical procedures, such as joint injections, removal of lumps and moles etc. This is usually done at Selby hospital on a Monday or Wednesday morning. Before any such procedure can be undertaken it is   important you see a doctor in surgery first. 

 

 

Patients over 75 years of age
We offer check ups for patients over 75 years of age. All patients aged 75 or over who have not been seen in the last 12 months will be written to and offered an appointment with the Practice Nurse. If you would like a check up please contact the surgery to arrange a convenient time.

 

 

Stop Smoking Groups

If you want to give up smoking but feel you need help, you can get free expert advice, support & information from North Yorkshire Stop Smoking Service by telephoning 0845 877 0025. If you would like to stop smoking and wish this service to help, you do NOT need to see a GP, you only need complete a self referral form.

From May 2008 Selby has a new facility for those seeking help in stopping smoking. A drop in service is now available at Morrisons Supermarket, Market Cross, Selby.

Travel Clinic
Most travel vaccinations, along with more comprehensive travel advice, are available at the surgery you usually use. These clinics are run by the Practice Nurses and are by appointment only.  In order for us to fully assess the need for travel vaccinations we ask patients to complete a form detailing their travel arrangements prior to making an appointment (see below)

Well Baby Clinics
These are run by the Health Visitors and run on the following days/times.

Beech Tree Surgery Mondays, 1.00 - 3.00pm 
Carlton Surgery the 1st and 3rd Thursday of each month, 10.00 - 11.00 am
Riccall Surgery the 2nd and 4th Wednesday of each month, 2.30 - 4.00 pm

 

Well Woman Clinic
These clinics are run by the Practice Nurses, we recommend that you contact your usual surgery to establish the time of available clinics. Health promotion is a major part of these clinics that also include smear tests and blood pressure checks.
